Acceptable Forms of ID
In accordance with 10 ILCS 5/4-10, two (2) forms of identification are required, one of which must include your current residence address. Please contact the Peoria County Election Commission if you are unsure if your forms of identification are acceptable. After review of the identification presented, the Election Staff has the right to make the final determination on acceptability.
Identification shall include, but is not limited to:
Valid Driver’s License
Valid Passport
Social Security Card
Birth Certificate
Current W-2 or 1099 form
Checkbook
Document issued by a Government Agency (showing name and address)
Business Mail or First-Class Mail postmarked to applicant
Online Utility Accounts
Online School Record Accounts
Lease or Contract for Residence
Identification Card with name printed or embossed:
Employee ID Card
Student ID Card
FOID Card
Concealed Carry License
Insurance Card
Medical Card
Link Card
Public Aid Identification Card
Selective Service Card
Civic Membership Card
Fraternal Membership Card
Union Membership Card
Professional Association Membership Card
Recent Financial Statement (online or paper-based; recent is defined as within the last 90 days)
Bill
Paycheck
Bank Statement

Utility bills in the name of one spouse may be used as proof of residence for both husband and wife.
Registering to Vote as a Homeless Individual:
A homeless individual must have a mailing address in order to be eligible to register to vote. A mailing address shall constitute a homeless individual's residence for voting purposes. A mailing address of a homeless individual may include, but is not limited to, a shelter, a day shelter, or a private residence (10 ILCS 5/3-2). The mailing address may be demonstrated by a piece of mail addressed to that individual and received at that address or by a statement from a person authorizing use of the mailing address. (10 ILCS 5/4-10).
